We are really exited to launch our new ENGAGE patient portal. An invitation needs to be created by either your GP or our reception staff. If you have already set up your account you can log in to your portal HERE
Please note all new patients will need a long (30 minute) consultation. Please call our reception staff via phone (02 4943 6511) to arrange your first consult. You will be forwarded a New Patient Registration Form via email to complete prior to your initial consultation.
If you would like (or have been asked ) to upload a file or form to your records please do so below. Note these files are not reviewed by our staff and are uploaded directly to your records sight unseen.
